Jurong Medical Centre (JMC) is a patient-centred health hub providing services that not only meets patients' needs, but also empowers patients to care for themselves, by offering knowledge and support.

JMC offers a range of services including outpatient specialists, diagnostic and day surgery services, chronic disease management, health screenings and promotion programmes.

Chronic Obstructive Pulmonary Disease (COPD), a lung disease characterised by chronic coughs, breathlessness and tiredness, is ranked seven in Singapore as the leading cause of death with over 10,000 hospitalisation episodes in a year. To help COPD patients access the care they need, improve their quality of life and delay or prevent disease progression, JurongHealth has launched the first of a national level series of Integrated Care Pathway (ICP) for COPD to customise and coordinate treatment across the care continuum from GPs to hospitals to the community.
